To remain on the active EIASE substitute list, please complete and return all required annual forms no later than June 15, 2026.
PLEASE NOTE - These annual requirements and the June 15 deadline also apply to the Preschool Screening Staff
What You Need to Complete:

Complete the Intent to Sub Form
Complete the form in full.
Review Title IX Information & Complete the Agreement Form
Review the required Title IX information, then sign and return the Title IX Agreement Form.
You do not need to return the questionnaire.
Complete the DCFS Mandated Reporter Training
Complete the required training and submit a copy of your updated certificate.
Important: Your certificate must be dated June 1, 2026, or later.
Complete the CANTS 22 Form
Sign and date the DCFS Acknowledgement of Mandated Reporter Status (CANTS 22).
Completed forms and required documents may be submitted by email or by mail.
